| NOTICE: Once orders are filled and mailed, no
substitutions of Materials or Tests for an individual will be allowed.
No issuance of Materials or Tests to individuals that we do not know about
will be allowed. As of January 1, 2010, all Tests will be marked "Do Not
Copy", which means exactly that. Exceptions to these policies must
receive written or verbal
permission from Quality Assurance Consulting, LLC,
in advance of our accepting any variations from a correctly submitted
Continuing Education Order Form. PLEASE LIST ONLY ONE "STUDENT" PER FORM, to avoid confusion of the ordered materials. |

Registration Information
We will send specified materials and a Test for each. For Continuing Education Credit, complete the Test(s), and send them to the above address for grading. Test(s) that receive a passing score will be rewarded with Certificate(s) of Completion by mail. If any Tests do not receive a passing score, we will notify and work with you to offer additional assistance with the goal of successfully completing a specific test. (Note: there can be no guarantee of successful test completion.) NOTE: No Materials will be mailed out until the properly completed Application Form and payment are received. Payment covers materials, shipping, handling, testing and certification. Additional offerings may be available in the future. All Providers, Resident Managers, Administrators and Caregivers (Certified and non-Certified) working in Adult Family Homes, Boarding Homes and DDD Certified programs are required by D.S.H.S. (St. of WA) to have ten (10) Continuing Education hours (related to Health Care Topics) per year. This requirement will remain in force through 2010.
